Microsoft Forms is an online survey and form creation tool that lets users create surveys, quizzes, polls, and questionnaires for various purposes. Microsoft Forms provides a user-friendly interface for creating forms without requiring advanced technical skills. Who is eligible?

Microsoft Forms is available to all users with a Microsoft 365 license assigned to their University e-mail. 

Where and how do I access it?

Microsoft Forms can be accessed by going to https://forms.microsoft.com or selecting the application from the Microsoft 365 application list found at https://portal.office.com. To login, please use your Sacred Heart University e-mail and password.
