Who is eligible?

To fill the form, the user might or might not need to sign in with SHU username/password to actually submit the form. To request a form to be created, a client will need to submit a service request.

Where and how do I access it?

Each form has a unique URL where it can be accessed.

SHU username and password will be needed to sign forms that require digital signatures.
